Advancements in Treating Diffuse Midline Gliomas

Researchers have made significant advances in the study and treatment of diffuse midline gliomas. Once seen as a terminal condition for affected children and adolescents, these developments bring new hope.

Ethan White, a member of the University of Michigan marching band, noticed something was wrong during drum practice. Unable to feel his right hand gripping the drumstick, he later experienced a fall on stairs. The university’s health service advised a neurology consultation. In March 2024, Ethan, then 20, was diagnosed with the aggressive brain tumor known as diffuse midline glioma (D.M.G.). This cancer predominantly affects children and adolescents, but adults can develop it too.

D.M.G. is known as the most lethal pediatric cancer. Historically, children diagnosed with it survived only about a year, with limited treatment options. Radiation could lessen the tumor’s growth and improve symptoms like weakness and double vision, yet survival rates remained grim. Patients typically succumbed to the disease in a short time.

In recent years, research has greatly advanced. Scientists have identified a genetic mutation common in many D.M.G. cases and are testing therapies targeting these genetic defects. The new treatments do not provide a cure, yet they have succeeded in slowing the disease’s progression for some patients. A small number have survived for several years.

“For the first time, we don’t get zero survival,”

said Dr. Sabine Mueller, director of the Pediatric Brain Tumor Center at the University of California, San Francisco.

In 2025, the F.D.A. approved the first drug for D.M.G. Although other treatments remain experimental, research continues. Ethan is now in hospice care, surrounded by the full-time support of his parents.
