An alarming report from the Lancet medical journal highlights the dire situation in regions like Sudan and Gaza, where civilians face extreme hardships and overburdened hospitals. Co-author Dr. Paul Spiegel emphasizes that the current humanitarian aid system fails to address modern emergencies effectively.

Dr. Spiegel, who has over 30 years of experience in refugee camps and conflict zones, considers the global response system inadequate for today’s crises. He reflects on Sudan’s severe civil war, noting that tens of millions require assistance amidst widespread hospital closures and famine. Despite the knowledge on how to save lives, the system struggles to deliver, according to the report.

The experts behind the report, titled Health in a World of Crises and Impunity, criticize some organizations for slow responses and excess bureaucracy. They call for global reforms, citing the need for significant changes within the United Nations. In particular, the report addresses the U.S. decision to close the United States Agency for International Development (USAID) during the Trump Administration. The closure, prompted by concerns over fraud, negatively impacted several vital programs.

Dr. Spiegel points out the necessity of restructuring USAID and the United Nations with care to avoid harming vulnerable populations. He stresses strategic planning to ensure aid reaches those in need without disruption.

The authors advocate for comprehensive changes, proposing direct aid delivery to local communities, enhanced accountability measures, and the recognition of healthcare as a fundamental human right. Dr. Spiegel calls for a ‘complete rebalancing’ to ensure the system supports its intended beneficiaries.
